The service will, on instruction of the Inclusion, Assessment and Review Team at Middlesbrough Council, write Education, Health and Care Plans (EHCP’s) in line with the SEND Code of Practice 2014. Middlesbrough Council will instruct the service and provide sufficient information to complete each plan. It is anticipated that approximately 30 EHCP’s - annual reviews requiring amendments, will be sent to the provider each month.
