Description

To support individuals to discharge safely on pathways zero and one by providing a range of support to facilitate the discharge, including but not limited to: transport to return home, 1:1 support, welfare checks, light meal preparation and domestic support, medication collection and referrals on to other support and services as required. This service will support the growing number of older and vulnerable people in North East Lincolnshire who are living with multiple comorbidities in the community who are at risk of hospital admission, or delayed discharges from hospital if their needs are not appropriately met by community provision. ## Contract award
