Description

The provider will work with its constituent GP practices, other community, voluntary and secondary care providers to provide a range of preventive and interventive health services including coordinated care for people with 3 or more long term conditions. The total lifetime contract value is £1,076,026. Lot 1: The notice relates to Population Health Services as described in section 11.1.4 above. This notice is an intention to award a contract to the existing provider following direct award process C. The lifetime value of the contract is £1,076,026. The contract will start on 01/10/2025 and end 31/03/2028. Additional information: This is a Provider Selection Regime (PSR) intention to award notice. The awarding of this contract is subject to the Health Care Services (Provider Selection Regime) Regulations 2023. For the avoidance of doubt, the provisions of the Public Contracts Regulations 2015 do not apply to this award. The publication of this notice marks the start of the standstill period. Representations by providers must be made to decision makers by midnight Tuesday 30 September 2025. This contract has not yet formally been awarded; this notice serves as an intention to award under the PSR. The provider was scored on a 0 - 4 scale and a % allocation based on importance of the criteria. The criteria for evaluation had the following weighting : 1) Quality and Innovation: 20% 2) Value: 20% 3) Integration, Collaboration, and Service Sustainability: 30% 4) Improving Access, Reducing Health Inequalities, and Facilitating Choice: 20% 5) Social Value: 10% The criteria were applied with their respective weightings as specified.
