Clinical Remote Patient Monitoring

Description

HIOW ICB have procured a single remote monitoring platform  for the management of patients at scale, and for patients to submit their health information securely, including the following core digital components:   Clinical platform/dashboard - enabling the remote monitoring service clinical team to access and provide real-time monitoring of patient data and RAG (red, amber, green) alerts 365 day per year.   Patient facing app/portal - ability for patients to input agreed health data with configurable symptom question sets and escalations.    Data and information sharing - interoperability and process for pertinent data to be populated into the local Connected Care Record (CHIE). Ability to run / extract activity reporting.   This has been awarded via a Mini-Competition through the SBS TECS2 framework,
