Description

Provision of enhanced practice nursing services and community based, multi-disciplinary healthcare for patients aged 65 and over within primary care networks in Harrow. The service will support improvements in health and wellbeing, enable patients to remain in their place of residence and will be flexible and adaptable, collaborating with other services and transitioning into integrated care teams. Direct award of a contract for a period of 36 months to enable Harrow CCG to deliver this service through the local primary care infrastructure in Harrow, ensure standardisation across primary care and work towards the development of a fully integrated care service across Harrow. Recognising its obligations and responsibilities under the NHS (Procurement Patient Choice and Competition) (No. 2) Regulations 2013, the CCG has assessed the most appropriate route to awarding this contract, having regard to the necessity to secure continuity of these essential patient services and has concluded that the incumbent provider will assure the safest solution and is considered to be the only technically capable provider at this time.
