End of Life Care Support Service - Call Off from Home Care Services DPS - AWARD

Description

Suppliers who have been successful in being admitted onto the Home Care DPS are being invited to submit tenders for the following call off contract for End of Life Care Support Service. The service will provide support for 'End of Life' (EOL) patients, as identified by the District Nursing Service, where a Continuing Health Care (CHC) package is yet to be put in place in place and/or where there are no family member or friends available to provide the necessary care.  The Council proposes to enter a Contract for an initial period of 2 years with the successful tenderer (Service Provider), with no option to extend Contract Value: (£398,205.88 per annum) Providers are not required to submit any pricing schedule for this block contract.
