Pan-Camden outreach bus, focussed on the most deprived wards, with the aim to reduce the barrier of access to healthcare for people in these areas by partnering with Camden council. The Outreach bus collects info on: smoking status, BMI, blood pressure, Know Your Risk score. Clinicians onboard have conversations around diabetes/pre-diabetes risk, whether they are registered with a GP (and encouragement to do so), as well as providing onward follow ups with GP for diabetes/hypertension/NHS check, other health interventions and sign posting towards council and VCS supported H&WB initiatives. Lot 1: This notice relates to Pan-Camden outreach bus, focussed on the most deprived wards, with the aim to reduce the barrier of access to healthcare for people in these areas by partnering with Camden council.
