Description
We require a provider of specialist health services for people with a learning disability resident in Tower Hamlets. The service will deliver high quality, personalised and safe services to meet the needs of people in transition, adults and older people with learning disabilities, and those who display behaviours that challenge. The service will be delivered to those living independently, with families, in residential care homes, supported living and in hospitals. Services will need to proactively address the health inequalities experienced by people with a learning disability and ensure access to the service which best meets the individual’s needs, regardless of the presence of a learning disability; including mental health, community health, primary and secondary care services. The perspective supplier will need to work closely with Tower Hamlets Borough Council and the staff employed by them in the service.Services will develop interventions that take into account and effectively address nationally identified causes of premature deaths of people with a learning disability. Additionally, playing a vital role in and working in close partnership with allied health professionals to ensure strong networks and clear pathways to support people accessing healthcare services. Reflecting the direction of the Transforming Care agenda commissioners expect a significant shift of focus towards robust community based interventions, ensuring that appropriate support is in place for whose behaviour challenges within the learning disability cohort, and the need for specialist hospital admissions is minimised. Services will develop interventions that take into account and effectively address nationally identified causes of premature deaths of people with a learning disability. Additionally, playing a vital role in and working in close partnership with allied health professionals to ensure strong networks and clear pathways to support people accessing healthcare services. Reflecting the direction of the Transforming Care agenda commissioners expect a significant shift of focus towards robust community based interventions, ensuring that appropriate support is in place for whose behaviour challenges within the learning disability cohort, and the need for specialist hospital admissions is minimised. Services will need to be flexible and responsive and able to provide intensive proactive support, which is not restricted to office working hours. Where the intensive support in the community is unable to continue to treat an individual at home and in-patient assessment and treatment beds are required a multi-disciplinary approach to appropriate, effective and swift discharge will be taken. The aim of the service will always be to focus on improvement, recovery and returning a person to their home setting wherever possible.
