The aim of the service is to provide a comprehensive primary care clinical information and facilitation service for NHS City and Hackney CCG in conjunction with all GP Practices to monitor service deliver and improve clinical outcomes for patients in primary care. City and Hackney CCG is undergoing a merger with 6 other CCGs on 1-Apr-2021 and is intending to issue a short extension (12 months) to all contracts expiring on 31-Mar-2021 to provide continuity during the merger. This is under regulation 72(b)(i) and 72(b)(ii) of the Public Contracts Regulations 2015 because a change of contractor cannot be made for technical reasons and would cause significant inconvenience for the CCG." Lot 1: Primary Care Information and Facilitation aims to provide a comprehensive primary care clinical information and facilitation service for NHS City and Hackney Clinical Commissioning Group in conjunction with all GP Practices to improve clinical outcomes for patients in primary care. The service provides detailed support including on-site teaching and facilitation programmes to improve quality across practices uptake of clinical guidelines, design and facilitation of templates for clinical systems and decision support tools. Supporting practices with implementation and audits. The service will provide dashboards, statistical analysis and interpretation of clinical data to meet the requirements of NHS City and Hackney Clinical Commissioning Group as commissioners. The Referral Form Management aims to centralise referral form management across health and social care services in City and Hackney in order to improve consistency, quality, efficiency and effectiveness of referrals.
