Tenders are invited for the delivery of the Aros Byw Bod Adref project. This is a time-limited, medium intensity floating support project for people who are experiencing early signs of dementia, as well as people who could potentially experience difficulties in maintaining their independence due to memory problems, depression, psychosis, or similar issues. Individuals supported will be resident in Denbighshire, either homeless or at risk of becoming homeless, and requiring support to enable them to manage their accommodation independently. Support will be time limited at a maximum of 2 years. The project will support a minimum of 10 people at any one time. This is a 3 year contract, with the OTE for 2 years (the total contract value given within this notice is for the period of 3 years). Note: DCC is using Proactis to carry out this procurement process. This is a separate website (https://supplierlive.proactisp2p.com), which requires registration to view tender documents and submit a bid. Individuals supported will be any gender; aged 16 and above; single or part of a couple, and may or may not have dependent children. All individuals supported will be resident in Denbighshire, either homeless or at risk of becoming homeless, and identified as needing support to enable them to manage their accommodation independently. Support will be time limited at a maximum of 2 years, as appropriate to individual needs — after which time individuals supported would be expected to have developed the skills to independently access any ongoing support they may need. The project will support a minimum of 10 people at any one time. Support worker hours will be managed dynamically and will be allocated flexibly in response to support needs. The project will be expected to have the capacity to accept urgent referrals as may be required from time to time. Staff will deliver support activities in appropriate settings in the community, including the homes of individuals supported. The contract is for a period of 3 years, with the options to extend for 2 years (the total estimated contract value given within this notice is for the period of 3 years).
