Description
The Pacesetter Programme, a comprehensive range of initiatives, has been running across all seven Local Health Boards in Wales. The Pacesetters, funded by Welsh Government, aim to stimulate innovation and promote the redesign of primary care services with pace, with a view to learning from those which deliver benefit and to share successes across Wales. The programme has been effective in bringing together primary care colleagues from around Wales to share the learning and to develop an emerging model for whole-system change across the NHS in Wales. Primary Care stability lies at the heart of this emerging model and is essential to ensure that GP practices and clusters are sustainable and can respond to future demands. A holistic Multi Disciplinary Team model within primary care offers a more proactive and preventative approach to care and is highly likely to reduce attendance at emergency departments and avoid admissions, thereby allowing staff in hospitals to focus their resources on the very sick and on planned specialist care. This reflects findings of international models in which strengthened primary care teams provide same day access, thereby reducing referrals to secondary care and unscheduled care admissions. This tender is to undertake an evaluation of 3 elements of the project: 1. Economic Evaluation 2. Patient Experience 3. Project Design We are looking for an experienced provider to undertaken the evaluation of all 3 elements. We will expect the provider to: 1) Make recommendations for the methodology to be used for each element 2) Provide a time line for completion of the evaluation of each element 3) Provide a date for the presentation of the findings via a formal presentation and through the submission of a final written report 4) Share all materials and data gathered in the process of undertaking the evaluation, noting that all finding and evidence gathered will be property of the commissioning bodies and not that of the providers, this should be shared electronically at the same time as the final report
