The award of contracts for the delivery of NHS Health Check Services from 1 April 2026 to 31 March 2030 Lot 1: The award contracts for the delivery of NHS Health Check Services from 1 April 2026 to 31 March 2030 to the nine Primary Care Networks (PCNs). This is a Direct Award C the NHS Provider Selection Regime to the following 9 Primary Care Trusts and GPs that form them • AT Medics PCN: Streatham High Practice and Edith Cavell Practice • Brixton and Clapham Park PCN: Clapham Park Group Practice, Hetherington Group Practice and Hetherington at the Pavilion. • Clapham PCN: Clapham Family Practice, Grafton Square Surgery, Dr Curran & Partners, and Sandmere Practice. • Croxted PCN: Paxton Green Group Practice, The Old Diary Health Centre, and Brockwell Park Surgery. • Fiveways PCN: Vassall Medical Centre, Akerman Medical Practice, Minet Green Health Practice, Herne Hill Road Medical Practice and The Corner Surgery. • HBD PCN: North Wood Group Practice, Herne Hill Group Practice, Brixton Hill Group Practice, the Deerbrook Surgery, Prentis Medical Centre, and Knights Hill Surgery. • North Lambeth PCN: Waterloo Health Centre, The South Lambeth Road Practice, The Vauxhall Surgery, Mawbey Group Practice, Lambeth Walk Group Practice and Hurley and Riverside Practices. • Stockwellbeing PCN: Binfield Road Surgery, Stockwell Group Practice, Beckette House Practice, Springfield Medical Centre, The Grantham Centre Practice. • Streatham PCN: Valley Road Surgery, The Exchange Surgery, Palace Road Surgery, The Streatham Hill Group Practice, Streatham Common Group Practice, and the Vale Surgery. Additional information: The existing provider is satisfying the existing contract and will likely satisfy the proposed new contract, and the contract is not changing considerably
