Description

To support the appropriate discharge of SWYPFT patients who are facing delays due to challenges around their housing in the community e.g. no accommodation or a return to their home may be compromised for a number of reasons. Also supports the tenant to resettle into the community and aid them in finding a more long-term housing solution. Lot 1: The Contract Authority, NHS West Yorkshire Integrated Care Board intends to award a contract to an existing provider following Direct Award Process C.
