Specialist Prevention and Intermediate Care (SPIC)

The proposed SPIC services will typically be for people with support needs related to mental health, drug/alcohol dependency, homelessness, autism and/or learning disabilities. The proposed aims of the services are to: • Enhance daily living and practical skills, to promote independence and prevent escalation of health, care and support needs • Improve housing stability and safety, and build emotional resilience, to prevent homelessness, repeat homelessness, or housing-related crisis • Improve mental and physical health, wellbeing, and social inclusion, to address wider determinants of health and support long term recovery. There are three service areas within the proposed model: • Housing and Daily Living Support – short term floating/community outreach support • Mental Health Recovery and Reablement - short term floating/community outreach support • Homeless - Out of Hospital Care / Social Care Crisis – short term supported accommodation Keywords: Intermediate Care, Specialist Prevention , Homeless and out of hospital care, Community Outreach, Day Support
