Home from Hospital Service

Description

The Home from Hospital Service provides short-term, flexible support to Liverpool residents who are being discharged from hospital and require assistance to safely return home, regain independence, and reduce the risk of readmission. The service forms part of the wider discharge-to-assess and community support pathway, helping people transition from hospital to home in a timely and person-centred way. The service is expected to support adults who may be experiencing temporary frailty, illness, reduced mobility, or diminished confidence following a hospital stay. Interventions are generally delivered for a time-limited period and focus on enabling recovery, promoting independence, and connecting individuals with longer-term community, health, social care, and voluntary sector support where appropriate.
