Pseudo Dynamic Purchasing System (PDPS) for Care in the Community. The PDPS is split into the following Lots: Lot 1 - Homecare, Standard; Lot 2 - Homecare, Complex; Lot 3 - Supported Living, Standard; Lot 4 - Supported Living, Complex; Lot 5 - Extra Care Housing; Lot 6 - Specialist Community Support The Contract shall commence on 16th September 2025 and end on 15th September 2028. The Authority has the option to extend by maximum of 6 years and 9 months in any increments of the Authority's choosing at its sole discretion (total possible term of 117 months). Lot 1: Standard Homecare Homecare is considered any care and support a person might require in their own home, including preparing meals, taking their medication, and attending their personal care needs. The Service Provider will adhere to the following Service aims and requirements throughout delivery of the Services for the following; • Older adults • Adults of all ages with organic mental health problems including dementia (includes those who exhibit behaviours that challenge) • People who may be physically frail or with physical disabilities • People who have sensory loss • People who are entering the end stages of life • People with learning disabilities • People with autism or other diagnosed neurodiversity • People with cognitive impairment • Discharge & Transition Service • People who are entitled to Continuing Health Care funding • People in need of specialist health tasks In addition to the above, support may also provided to family carers with any of the above disability, subject to to a carers assessment as part of the Care Act 2014. The service provider is to support people who are living at home, and to focus on achieving their agreed outcomes. These People may have been through a period of reablement with another care provider, and strong links will need to be established between Providers to ensure the smooth transition of care/support for People where appropriate, ensuring People continue to be empowered and enabled. This includes day time care, night time care, waking nights, sleeping nights and live-in care. Full detail of the scope of the requirements can be found in Appendix 1a - Specification Lot 1 The Contract shall commence on 16th September 2025 and end on 15th September 2028. The Council's Social Workers / Adult Support Co-ordinators are responsible for assessing the need for Complex Homecare Services and defining the Service required in the individual's Care Plan. Complex needs are defined as a Person assessed as having a learning disability, neurodiversity and / or mental health conditions and presenting with multiple conditions that necessitate specialised care. In these cases, standard interventions are insufficient and require specific expertise and a bespoke, multidisciplinary approach to care. Full detail of the scope of the requirements can be found in Appendix 1b - Specification Lot 2 The Contract shall commence on 16th September 2025 and end on 15th September 2028. Lot 3: Standard Supported Living Supported Living is typically defined as housing where support and or care services are provided to help people live as independently as possible, and is usually commissioned for individuals with learning disability, neurodiversity or mental health needs Supported Living is a person-centred approach to integrated housing and care and support services. It is based on People having a tenancy or license agreement offering the right to remain in their own home and change their support provider if they wish. It can mean living in shared accommodation or living alone with care and support. The aim of Supported Living is to support Peterborough City Council's strategic objective to help people to improve or maintain their health and well-being and to live as independently as possible. A Supported Living service supports people to develop key independent living skills and enables them be an integral member of their local community. Full details of the service scope can be found in Appendix 1c - Specification Lot 3. The Contract shall commence on 16th September 2025 and end on 15th September 2028. Lot 5: Extra Care Housing Extra Care Housing (EHC) are specialist housing schemes for older people that have been specifically designed to maximise independence. All tenants have their own apartment with a front door and yet also benefit from the availability of the 24/7 onsite care and support service. Extra Care Housing provides an integrated housing, care and support solution whereby people can benefit from having care and support services available to them within the housing scheme they live in. Service Users will be enabled to: remain in their own homes, with a service that meets their varying and increasing care needs; access appropriate community facilities; at all times remain as independent and self-caring as possible. Extra Care Housing should provide a community where people aged 55 and over with different levels of capabilities can be supported. Not all Service Users will have a care need, but they may benefit from living in a more supportive environment. All Service Users benefit from the additional security of having a Waking Night Support service. Some Service Users will be Service Users of the Council and will have formally assessed care and support needs. Tenancies will be allocated in accordance with this ethos of promoting a 'balanced community' within Extra Care Housing. The full scope of the service is detailed in Appendix 1e - Specification Lot 5. The Contract shall commence on 16th September 2025 and end on 15th September 2028. The Authority has the option to extend by maximum of 6 years and 9 months in any increments of the Authority's choosing at its sole discretion (total possible term of 117 months). Lot 6: Specialist Community Support Where a person has eligible care needs and requires support in addition to standard or complex care at home, services can be called off from this Lot. This support can include, but is not limited to, the below: Individual support: • Where a Person may need rehabilitative support for a short period of time. This might include following a stay in hospital, recovering from a stroke or acquired brain injury. This might include supporting them in relearning day-to-day living skills and working together for the person to become more independent. • Where a Person may need support in the community to go shopping, attend groups or activities, and achieve more holistic outcomes. • Where a Person may need support at home that is not considered 'typical' homecare. The Person might be able to meet their own personal care needs, but have additional needs outside of the home or living independently. Floating support: • This might include a support worker who delivers day-to-day support to a group of People living in an area. • It may also include someone who needs to be specially trained and have an understanding of a specific People's (ie client group's) needs. • It may be location based or building based, but can include a number of different People being supported to achieve specific outcomes. The full scope of the service is detailed in Appendix 1f - Specification Lot 6. The Contract shall commence on 16th September 2025 and end on 15th September 2028.
