Connecting Communities - Mobile Social Prescribing Services

Suffolk County Council is seeking a delivery partner for social prescribing services. The Council is part of an EU funded programme to develop local community-based services to improve the health and wellbeing of older populations affected by rural and social isolation and deprivation. <br /> <br />The programme will create a mobile social prescribing 'plus' scheme which will have the same components as a traditional social prescribing scheme but will be sited within a wider framework of early intervention and prevention services. <br /> <br />The provider will be expected to work closely with East Suffolk communities teams and SCC localities and partnerships teams to identify and develop a network of local service provision primarily within the non-statutory sector but linking to statutory bodies as appropriate. <br /> <br />The tender submission deadline date is 23 October 2019 at 12 noon. <br />
