Live Well Community Navigation Service

Description

Tameside Live Well Community Navigation Service is a new community-sited and community-delivered service to support the health and wellbeing of local residents. Funded by Tameside Council, the Live Well Community Navigation Service will address these challenges by connecting people to community-based support that improves wellbeing and reduces demand on statutory services. Although there is a range of good quality, individualised support available in Tameside, it can be difficult to be aware of what is available and to navigate it, whether you are an independent resident, or one that struggles to access support easily. An effective community navigation service, with strong neighbourhood-level links to the range of statutory and community-based groups and services is a powerful tool to ensure more people living in Tameside can get access to the right help and support at the right place and time to ensure a positive impact on their lives, and to prevent further harm developing. To complement the on-the-ground navigation support as part of this service, the funding envelope also includes resources for community development which is a key aspect to gain an in-depth understanding of the community assets and gaps across Tameside. This will inform where further investment and development may be needed, in order for the VCFSE sector to meet the needs of residents when they are supported to further help by community navigators, or via other routes. The evidence tells us that the value and impact of these types of roles includes (NHS England, 2020): • strengthening community resilience • reducing health inequalities • increased involvement in local communities • giving time to people to focus on what matters to them • often have strong knowledge of their local community groups Please Note: The Council places a huge importance on Social Value.
