FEW1486 - NHS Health Checks Service

Description

The NHS Health Check programme is aimed at detecting those most at risk of Cardiovascular Disease (CVD) and will be offered to patients aged 40-74 years old. This programme is provided by GPs in Leicester City who are given a payment for every patient seen and if a patient is placed on a management plan a further payment is given. A patient who is assessed as not needing to be placed on a management plan, then the patient is recalled to have a further check every 5 years until either they are ineligible through age (or in the intervening time placed on a disease register for a condition). In the event that a patient is assessed as needing to be placed on a clinical pathway to have their conditioned managed then a one-off payment is given to the GPs, and the patient is no longer invited for a Health Check. This contract is awarded through Direct Award Process B under the Provider Selection Regime. Full detailed requirements can be accessed through the published suite of documents. The lifetime value of the contract is £1,827,000.00. The total contract term is 7 years with an initial term of 5 years commencing from 1st April 2026 - 31st March 2031 with the option to extend for a further period(s) of 2 years subject to funding and satisfactory performance, until 31st March 2033. This is an existing service with existing providers.
