Young Peoples Supported Living/Housing Pathway

Description

A multi-lot framework agreement aimed at meeting the support needs of young people leaving care and vulnerable young people facing homelessness. The accommodation pathway is designed to provide move-on from care or home setting into a Pathway offering assessment, mediation, guidance, training and support leading to independent living. The step-down accommodation units comprise of shared houses and self-contained flats where support workers are allocated to each young person throughout their stay. The framework consists of 5 LOTS providing each stage of accommodation service meeting the needs of our young people. Lot 1 – Assessment, Support and Mediation Service 18-25 year olds Lot 2 – Supported Housing 16yrs to 25 includes Teenage Parents and UASC Referrals Lot 3 – Supported Housing 16yrs to 25 to include UASC Referrals Lot 4 – Transitions / Step-Down 18yrs to 25 includes Teenage Parents Lot 5 – Floating Support Service (30 spaces) Keywords: Young People
