NHS Bristol, North Somerset and South Gloucestershire (BNSSG) Integrated Care Board (ICB) is intending to undertake a procurement to commission Primary Medical Services for the population of patients registered with Graham Road Surgery and Horizon Health Centre in Weston-super-Mare to replace existing APMS contracts expiring in the Summer of 2026. The planned Invitation to Tender (ITT) will be for the delivery of services across the two sites, under one APMS contract, with a contract term of 15 years, in line with specified requirements. This notice is placed as a Prior Information Notice (PIN) to seek expressions of interest and a short question set to gain input and insight from providers in the market, which will inform the ITT process. Interested parties should register their interest and complete their market engagement question responses by mid-day (12:00) 10th July 2025. This exercise is being carried out by NHS South, Central and West Commissioning Support Unit (SCW) on behalf of the Commissioner. Branch site: Horizon Health Centre, Healthy Living Centre, Weston-super-Mare, BS23 3SJ The practices are situated in extremely deprived areas of Weston-super-Mare. The aims of the service will be to: • Deliver an operating model that responds to the specific population needs • reduce health inequalities within the population • Provide robust a robust workforce and leadership model which meets the needs of the population • Provide value for money • Deliver universal Primary Care Services to the registered patients of the two sites • Provide continuity of care for the 15,420 registered patients (Graham Road Surgery - 9,741, and Horizon Health Centre - 5,679) • Provide a Modern General Practice Model of Care • Work in an integrated way with other primary care services, Primary Care Networks, mental health and community services and all acute care services • Collaborate and engage with third sector organisations, the relevant Locality Partnership Board and other professional groups supporting residents, to increase uptake of health interventions to improve health outcomes for registered patients Background Graham Road Surgery is situated in the Weston-super-Mare Central ward of North Somerset, whilst Horizon Health Centre is situated in the Weston-super-Mare South ward. The practices are 1.6 miles apart, with a driving time of 6 mins, and a walking time of 29 mins . In Weston-super-Mare Central, 95.2% of the population are White, and 2.1% are Mixed or Multiple ethnic groups. 85.6% of the population's first language is English, whilst 11.7% is European languages. Life expectancy at birth is 74.8 years, compared to the North Somerset average of 82.3. Six Lower Super Output Areas (LSOA) make up the ward; average IMD decile is 1.8 , compared to North Somerset at 6.9. The percentage of people who reported having a limiting long term illness of disability is 31.4%, compared to North Somerset at 19.1%. In Weston-super-Mare South, 95.7% of the population are White, and 1.8% are Asian/Asian British. 92.4% of the population's first language is English, whilst 5.5% is European languages. Life expectancy at birth is 78 years, compared to the North Somerset average of 80.3. Six LSOAs make up the ward; the average IMD decile is 1.4 compared to North Somerset at 6.9. The percentage of people who reported having a limiting long term illness of disability is 24.3%, compared to North Somerset at 19.1%. It does not represent a guaranteed level of income. Graham Road Surgery and Horizon Health Centre have a combined weighted list size of 16,968 The estimate financial envelope is £3,440,994 per annum, meaning a potential total contract value of £51,614,910 over the intended 15-year term. Contract values are subject to quarterly list size adjustments, Global Sum and Out of Hours adjustments. Additional income is available from the Quality Outcomes Framework, Direct Enhanced Services, and Local Enhanced Services (LES), noting Local Enhanced Services commissioned by the ICB are offered on an annual basis and are subject to change based on local commissioning priorities. A non-recurrent payment may also be made available to support transition costs. Financial reviews are being undertaken by an independent specialist to support the financial resilience and sustainability of the contracts, and any recommendations from these reviews will be used when determining the final contract values. The ICB also wishes to engage with interested parties around the finances as part of this market engagement.
