Social Prescribing is an established model of care designed to enhance both mental and physical wellbeing. It forms a core component of Universal Personalised Care, empowering individuals to take an active role in managing their health through tailored, community‑based interventions. This approach supports a wide range of people, including those: - living with one or more long‑term conditions - requiring support with their mental health - experiencing loneliness or social isolation - facing complex social needs that impact their overall wellbeing Lot 1: Social Prescribing is an established model of care designed to enhance both mental and physical wellbeing.
