Description
Provision of a nurse-led, health inclusion service to the City of London Rough Sleeper Assessment Centre known as Snow Hill Court Lot 1: The overall aim of this service will be to ensure access to primary health care on an Outreach basis to the residents of the City of London Rough Sleeper Assessment Centre. The service will ensure 1 day per week (0.2FTE) input from a specialist Community nurse, which will involve attendance at Snow Hill Court 1 half day per week, with some follow-up and casework.
