Out-Patient Management of Long-Term Complex Patients Physiotherapy Service

To provide a dedicated outpatient and domiciliary based management service to patients with complex postural and physical needs which do not fall within the scope of other NHS commissioned services Lot 1: The Chaseley Trust will offer an outpatient and Domiciliary Physiotherapy Service delivered and / or supervised by a Specialist Neurological Physiotherapist with expert knowledge and management skills of neurological conditions, complex needs and postural management. A patient will attend the Chaseley Trust for assessment via a referral from ESHT out-patient department or community-based rehabilitation team. There will be some patients who will require ongoing long-term maintenance due to their clinical presentation and diagnosis. These patients will be reviewed regularly (at least every 3 months) by a qualified physiotherapist who will thoroughly re assess the patient and their needs.
