The Live Well Kent Strategic Partners — Porchlight and Shaw Trust — intend to recommission Mental Health Housing Related Support Services in Kent. The newly commissioned service will be called Live Well Kent Community Housing. By integrating the service into the substantive Community Mental Health and Wellbeing Service known as Live Well Kent, the Commissioners aim to create a more seamless mental health pathway. The service will support individuals to live independently through recovery-focused interventions, delivered in a safe and supported environment. In addition, through promotion and facilitation of community participation of people who experience mental health needs, the service will aim to sustain continued recovery in move-on from accommodation and resettlement into the community, with support from the Live Well Kent network.
