This unified solution will create a single patient record across acute, community and mental health care settings. This single solution with strong interoperating capabilities will enable seamless working with primary care, social care, and other partners, providing a single view of the patient and enable maximum sharing between partners. We are seeking to appoint a Delivery Partner who will have a "seat at the table" with a joint level of ownership and accountability to the successful delivery of the programme. We expect the Delivery Partner to bring the expertise, methods, processes and materials to enable the trust teams to acquire the right skills, to be supported and steered through a complex programme that requires significant forward planning and preparation. They will also bring insights methodologies and skills to drive forward the clinical and operational involvement as well as the technical skills. They would be integrated into the local team with a clear expectation that the local team will gain from transference of skills and knowledge to ensure the trusts are self-supporting going forward.
