This is a Request for Information (RFI) published on behalf of NHS North East Essex Clinical Commissioning Group (CCG) With this RFI, NHS North East Essex Clinical Commissioning Group is seeking to gain a better understanding of the current capacity of the market in relation to arranging appropriate onward social care for patients who are self-funders for Social care from hospital and also assisting members of the public who are not eligible to social services funded care and so will be self-funding social care who are at risk of being admitted to hospital, assisting them to find appropriate onward social care, while offering choice and value and supporting people to appropriately remain in their own homes or a home of their choice to remain 'well' and in an environment that supports healthy living.
