Description

Stockport Council Adult Social Care is embarking on an exciting and ambitious development bringing the former St Thomas’ hospital site back to life and transforming the way that we promote independence and provide the best possible outcomes for people. The Centre for Living Well will be a new facility that pushes the boundaries of design and innovation to create modern home-like accommodation where people are able to regain their independence. It will be located alongside a new residential community developed on a site steeped in rich heritage, with refurbished listed buildings and new homes built to the highest levels of sustainability creating an affordable place to live that will attract a truly inter-generational community with all the benefits of Town Centre living. The Centre of Living Well will provide locally based, high impact services with a focus on: • step-up facility to prevent hospital admissions • step-down facility to accelerate hospital discharge through reablement • access for respite or short breaks • En-suite, studio and batriactric accommodation within 5 households across 3 floors, totalling 82 intermediate care, including supporting people with complex care needs and a diagnosis of dementia • A state of the art technology lab, training facilities, gymnasium and outdoor podium garden • Ground floor facilities for the community It is our ambition for the Centre to be a Beacon of Excellence for intermediate care and we aim to achieve considerable outcomes for the people of Stockport as part of our Live Well strategy, as well as greater efficiencies, and market sustainability. We can achieve this ambition by working together with health and care partners across Stockport to provide the best intermediate care within one location, with both clinical and reablement expertise available onsite and with access to the facility from both the community and from hospital. We are undertaking some initial market engagement to test our ambition with the wider care market and to hear your views on the best operating model. The council therefore wants to hear from care providers who would be interested in becoming a strategic partner in the scheme. We want to explore and hear your views on: • Financial Models • Partnership arrangements including consideration of a management type contract arrangements • Reflections on the Risks and Opportunities of the ambition Those interested in this exciting opportunity we would like to invite to one of our virtual Stakeholder Events on 5th June where we will deliver a presentation on our proposals concluding with a Q&A session. If you are interested in attending this session on Microsoft Teams, please contact Michelle Moseley, Programme Manager, email: michelle.moseley@stockport.gov.uk to book your place on this. Please do not use the messaging facility on The Chest as messages for this are not monitored and will not be responded to. Keywords: intermediate care
