A group of patients that have been identified as using a high proportion of NHS services, accounting for using approximately 10 % of total NHS expenditure on services for the working age population in England are people with medically unexplained symptoms (MUS) or persistent physical symptoms (PPS) (Joint Commissioning Panel for Mental Health, February 2017). The service aims to improve the symptoms and self-management of people with persistent physical symptoms. The patients seen by the service become not only more independent in managing their condition, using healthcare services less frequently, but are able to return to purposeful and meaningful roles at home and work, reduce carer burden and reliance on benefits. Expressions of interest are invited from suitably qualified and experienced providers capable of delivering the service in the County. The purpose of this service is to support the reduction of the use of diagnostic services and outpatient referrals, attendance at A&E departments and acute hospital admissions. The service aims to improve the symptoms and self-management of people with persistent physical symptoms. The patients seen by the service become not only more independent in managing their condition, using healthcare services less frequently, but are able to return to purposeful and meaningful roles at home and work, reduce carer burden and reliance on benefits. The service uses: • a person-centred approach; • assessment and formulation based on a bio-psychosocial model; • an equal acknowledgement of mental and physical health components to the person’s presentation.
