The proposed project consists of change of use of Block A, Galway West Business Park, Knocknacarra, Galway, from specialist office use to use as an Enhanced Community Care Hub (ECC) and Primary Care Centre (PCC) (Knocknacarra andSalthill).The Project is the refurbishment and deep retrofit of an existing open plan administrative office space, with change of use to a non-acute community diagnostics and treatment healthcare facility. an Integrated Care Programme (ICP) for Chronic Disease (CD) and Older People (OP) specialist team Hub in Galway City West located at Block A, Galway West Business Park, Knocknacarra, Co. Galway, circa 3,750 sq.m. In addition to providing a base for these specialist teams (ICP, CD, OP), it will also provide a base for 2 no. Primary Care Teams (PCT x 2) thus supporting the rollout of the associated Community Health Network (CHN) services.
