ASC069- Provision of 6 hours support each week to work with x1 Service User (CD) to support her to maintain her tenancy, familiarise herself with the area, to locate and access medical facilities and register with GP and the Specialist drug and alcohol services in that area and to promote engagement with social care services. Service will also offer emotional support and support Service User to therapeutic sessions ran by Substance Recovery Services to look to support the trauma she has experienced in her life.
