Description
The aim of the well-being at home service is to provide personalised support to vulnerable residents in their own homes and to develop integrated support plans and packages that are based around their needs. As part of the new integrated care landscape, local partners are collaborating to commission and provide an Integrated Support Offer (ISO) to the residents of Waltham Forest. The services form part of a wider development known as the managed network of care and support. The service covered by this VEAT notice is known as well-being at home. Within the local area, commissioners and providers have been working jointly to develop the service since 2017/18. The aim of the intervention is to provide personalised support to vulnerable residents in their own homes and to develop integrated support plans and packages that are based around their needs. It is now well established that these non-clinical interventions can prevent deterioration in health conditions and delay the need for formal care, as well as significantly reducing admissions to hospital. In addition, the interventions have been shown to improve quality of life and empower individuals to take an active part in their community — which reduces social isolation and increases well-being. The service is currently delivered by a partnership of local providers and will be re-commissioned from 1.11.2019. The organisations that make up the collaboration of local partners include: — Waltham Forest Clinical Commissioning Group (WFCCG), — London Borough of Waltham Forest (LBWF), — North East London Foundation Trust (NELFT), — Barts Health (through the Frailty Programme), — Age UK Waltham Forest (Coordinating Provider for Wellbeing@Home), — Home Energy Efficiency Training Ltd (HEET), — Citizens Advice Waltham Forest (CAWF), — Metropolitan Thames Valley Housing Trust (MTVH).
