Description
The London Borough of Richmond is seeking to appoint a single Service Provider to deliver the Lifestyle Prevention service contract which will be comprised of the following service elements and will operate as a Hub and spoke model. Lifestyle Hub -The Lifestyle Hub will act as a central referral hub/single point of access (SPA) and will provide outreach services to support identification and referral. It will also market and promote the lifestyle services, encourage participation by eligible residents and motivate people to achieve their lifestyle goals. The Lifestyle hub will manage the locally commissioned services contract for NHS Health Checks with GPs and Pharmacies and all associated support activity. Spokes - The Lifestyle Spokes will consist of the following services; NHS Health Checks Outreach Programme - will aim to identify individuals at high risk of heart disease and other long term conditions aged 40 - 74. The Outreach Programme will supplement health checks delivered by GPs and pharmacies ensuring that those who may not come forward to these settings can be offered a health check by community outreach. Health Walks - will be available to all residents of the borough, including walks for families and adults. The aim of this service will be to encourage and actively recruit people whom were not frequently active and provide a range of walks for people of differing abilities. Weight Management - will be in line with NICE public health guidance (PH53) for a successful weight management service. The service will demonstrate success at delivering people with long term weight loss. The target population are people that are overweight and obese and also have an existing health condition. Exercise Referral - will aim to reduce the risk of long term conditions by encouraging greater physical activity. The service will be accessible to people that are sedentary and also have a health condition in line with NICE public health guidance (PH54). Walking Away from Diabetes - will offer a validated 3 to 4 hour educational programme and an accurate pedometer and step challenge intervention. The provider will be expected to provide all elements of the service modalities as one service. The contract will be for an initial period of 2 years commencing in September 2015 with the option of two further periods of up to 12-months each, at the sole discretion of the Council. The total estimated contract value is £686,000 per annum but no guarantee of this value is given or should be implied.
