The Aberdeen City Health and Social Care Partnership has decided to recommission the delivery of care and support services at home for adults and has adopted a locality based, outcomes-focused contract to achieve the following aims: - To meet individual and system wide outcomes, including the aims of the strategic plan - To ensure market stability - To deliver care and support efficiently and effectively, including through the utilisation of digitalisation and technology assisted care - To ensure delivery is aligned to the medium and longer term financial plan Lot 1: Provision of Care and Support Services at Home in Aberdeen North Locality Provision of care and support services at home to all adults in the Aberdeen North Locality. Lot 2: Provision of Care and Support Services at Home in Aberdeen Central Locality The provision of care and support services at home to all adults in Aberdeen Central Locality Lot 3: Provision of Care and Support Services at Home in Aberdeen South Locality The provision of care and support at home for all adults in the Aberdeen South Locality.
