Modification of existing contract to enable an increase in clinical capacity of the service to support proactive management of the rising risk cohort of patients. The additional capacity will include: community nurses – specialist respiratory and cardiology; mental health link worker; elderly care consultants; and specialist GPs. Modification of existing contract under Regulation 72(1)(b) for the provision of additional services by the original contractor that have become necessary and were not included in the initial procurement, recognising that another contractor: cannot be made for economic and technical reasons due to requirements of interoperability with existing services; and that involvement in another contractor would cause significant inconvenience and substantial duplication of costs for the contracting authority. In line with the national direction of travel for CCGs to have an ICP in place, the variation will include the provision of additional clinical capacity into the service to support the team with the proactive management of the rising risk cohort of patients. The additional capacity will include: community nurses – specialist respiratory and cardiology; mental health link worker; elderly care consultants; and specialist GPs. The Integrated care partnership (ICP) is collaborative networks of care providers, bringing together healthcare professionals (including doctors, nurses, pharmacists, social workers, and hospital specialists); the voluntary and community sectors; local council representatives; and service users and carers, to design and coordinate local health and social care services. Given the specificity of the service required and the requirements for patient record access, the ICP working in collaboration with primary, acute, community and voluntary sector organisations are the only technically capable provider for this service. Because the ICP is not a legal entity, harness care cooperative will act as the host and banker for the ICP in Harness locality, subcontracting with other organisations as and when required.
