Hywel Dda University Health Board has awarded contracts for the provision of Learning Disability Support Services. The aims of the service are: The aim is to support Service Users and/or Carers of all ages, with Learning Disabilities, whom also have mental health condition(s). To support Service Users and/or Carers to remain as independent as is possible, continue to live in their current homes and be integrated into the community to reduce the feeling of isolation. Providing a service which supports the needs of those in, at the risk of social/emotional crisis Act as a conduit to identify, broker, and signpost individuals to other help and support agencies as needed This provision is to support Service Users and/or Carers with Learning Disabilities regarding their anxieties about community integration, via virtual and face-to-face initiatives Encouraging those accessing the service to self-manage, gain confidence, and live independently, where possible A continued commitment to co-production of Community Mental Health services awarded across the HDdUHB geographical footprint Lot 1: Carmarthenshire Hywel Dda University Health Board has awarded a contract for the provision of Learning Disability Support Services.
