Lewisham borough have implemented a Joint Medicines Policy and Medicines Support for all patients under a care arrangement. The Joint Medicines Policy spans across Lewisham Adult Social Care and NHS South East London ICB (NHS SEL ICB) - Lewisham, in collaboration with University Hospital Lewisham (UHL)/ Lewisham & Greenwich NHS Trust (LGT), Lewisham Integrated Medicines Optimisation Service (LIMOS), the Local Medical Committee (LMC) and Community Pharmacy South East London (CPSEL) It provides a common policy and procedure and a single agreed assessment pathway for the provision of pharmaceutical and medicines support across the NHS and Social Care in Lewisham borough. The policy incorporates the Medicines Support Pathway, and the Lewisham Medicines Support Assessment Tool. The policy mandates that all domiciliary care providers attend training on Medicines Administration and the associated pathways for assessment of medicines support needs in Lewisham. Training offered aims to assist staff providing care to: 1) Better support their service users with tasks related to medicines administration and support. 2) Work collaboratively with members of the Multi-Disciplinary Team (MDT) to reduce the risk of avoidable medicines related ensure appropriate review of medicines. maintain independency where possible. 3) Facilitate step-down recovery to independent living. The training offered is generic training to supplement medication training that provider organisations may be offering top their teams and is aligned to the Lewisham Joint Medicines Policy and Pathway. Staff are mandated to attend at least once. For equity, training is to be offered to staff working in the setting of care homes for older adults and other staff groups such as Personal Assistants where agreed. The specification, as set out below, is the suggested Medicines Support Training required within the borough of Lewisham. However, is subject to change depending on local requirement. The Contract will commence on 01/04/2024 for an initial period of 1 year. The Contract may be extended for an additional period of 1 year, at the sole discretion of the Authority. Value of contract: £30,000 per annum. The indicative value range for the total contract value is between £30,000 for 1 year and £60,000 for 2 year. VAT inclusive, as applicable.
