Specific primary care services to marginalised communities in Bradford, based at the Bevan House Primary Care Centre, Bradford. Lot Lot 1: APMS contract, delivering primary medical care services to the homeless, asylum seekers and socially excluded groups A 3 year contract (with the option to extend by up to a further 2 years) commencing 1 July 2022 for specific primary care services to vulnerable marginalised communities in Bradford including the homeless and rough-sleepers. The provider must have excellent relationships with other key services for vulnerable individuals such as housing, drug and alcohol, mental health and street support interventions and will play a crucial role in the development of integrated services for this population group by collaborating with other NHS providers and non-NHS services. This service also forms parts of Bradford district and Craven CCG strategy to tackle health inequalities. During the contract term the provider will work with the commissioners and other stakeholders (including NHS and non-NHS providers) to create a broader service offer to the most vulnerable, focusing on wider health and wellbeing needs. This may include further redevelopment to ensure a healthcare facility where innovative sustainable services can be delivered from. Lot Lots 2: NHS Standard Contract that provides clinical support to the Pathway Team, Bradford Respite, Integrated Care and Support Service (BRICSS) and delivery of a Street Medicine Team) providing an inclusion A 3 year contract (with the option to extend by up to a further 2 years) commencing 1 July 2022 for specific primary care services to vulnerable marginalised communities in Bradford including the homeless and rough-sleepers.
