This Early Intervention Support Service (EISS) is designed to support families with emerging vulnerability, with the aim of de-escalating issues of concern, achieving sustainable change, promoting capability and capacity within families to resolve difficulties they are experiencing. This early intervention can prevent families from problems escalating and offset the need for statutory services intervention using the combined expertise of participating agencies. In this tender family refers to family or families who have children and young people aged 0-18, in their family context, including expectant parents, who are experiencing difficulties within Tier 1 and Tier 2 of the Hardiker Model of intervention. Lot 1: Lot 1: Specific geographic areas within the Belfast Health and Social Care Trust This Early Intervention Support Service (EISS) is designed to support families with emerging vulnerability, with the aim of de-escalating issues of concern, achieving sustainable change, promoting capability and capacity within families to resolve difficulties they are experiencing. This early intervention can prevent families from problems escalating and offset the need for statutory services intervention using the combined expertise of participating agencies. In this tender family refers to family or families who have children and young people aged 0-18, in their family context, including expectant parents, who are experiencing difficulties within Tier 1 and Tier 2 of the Hardiker Model of intervention. Additional information: The value in Section V.2.4 for this Lot includes the contract period of 5 years, plus the option to extend. for 2 x 12 months periods [2 years]. The value also includes the following - a potential increase in the value of services of 50% AND potential adjustments to the contract price to reflect the Consumer Price Index AND the potential additional extension clause of 3 x 6 months as provided for in the SS20a Service Specification - clauses 11.2.5, 11.2.1 and 18 respectively. In addition, this value also incudes, a potential uplift of £60,000 per Lot per annum. Note – there is no guarantee of uptake or minimum demand.
