Aims and Objectives of the Service • The Provider shall work within the general aims, objectives and scope of the Service detailed in this Specification, or otherwise notified to the Provider during the Contract Period. However, the Provider shall work flexibly to ensure the aims and objectives of the Service are achieved. • The Provider shall provide the Service in a manner that allows Service Users to live independently with any risk, either to themselves or others, minimised through assessment, management and support. • The Provider shall be capable of working with people with a varying level and variety of needs. • The Provider’s staff shall have the necessary knowledge, skills and experience of working with community organisations. • The Provider shall promote and demonstrate the benefits of physical activity to patients suffering from a range of physical and mental health conditions, i.e. obesity, high blood pressure, mild to moderate depression etc. and those who are not currently active. • The Service seeks to provide a range of exercise options in leisure centres for priority groups of low to medium risk clients, (including Phase 4 Cardiac Rehab), to develop the confidence, skills and motivation they need to become and remain physically active by: • Offering individuals with a range of medical conditions the opportunity to increase physical activity in order to improve their health and wellbeing; • Developing the confidence, skills, and motivation they need to become and remain physically active; and • Providing Phase 4 Cardiac Rehab for cardiac patients who are deemed to be low/medium risk and meet British Association for Cardiac Rehabilitation (“BACR”) risk stratification criteria. The aim of the contract is to achieve long-term increases in levels of activity of physical activity in referred patients and to encourage better health outcomes for these patients. The objectives of the contract are to achieve the following: • To publicise and promote the Service; • To provide the opportunity to exercise with the support of a qualified instructor; • To provide opportunities for Group exercise Courses that meet the needs of the Service User; • To provide opportunities for inactive residents in the Authority who have a pre-existing condition that would benefit from physical activity; • To engage with GP surgeries; • To achieve high levels of continued attendance of exercise sessions; • To signpost Service Users and Groups to other relevant services; • To support Groups to develop a plan for how they can continue to be physically active after the end of the course; and • To follow up patients at six (6) and twelve (12) month intervals to assess whether or not the Service User is still active and if not to give the appropriate advice and support to encourage them to be more active.
