NHS Southport & Formby CCG and NHS South Sefton CCG - Independent provider of Ophthalmology Services NHS Midlands and Lancashire Commissioning Support Unit (CSU) are working on behalf of NHS Southport & Formby CCG and NHS South Sefton CCG who wish to make a direct award for an Independent provider of Ophthalmology Services for patients living in the CCG areas. The services due to be provided are: • Diagnostic and screening procedures • Surgical procedures • Treatment of disease, disorder or injury This Notice is to confirm NHS Southport & Formby CCG's and NHS South Sefton CCG's intention to award a contract for an Independent provider of Ophthalmology Services to iSight Limited. This contract was awarded to commence on 1st April 2021 and is a 6-month contract with the option to extend by a further 6 months.
